Challenge
Outpatient pharmacy teams shared the same goal but not the same approach. Each worked its own way, leading to duplicated effort, uneven workloads, and inconsistent attention to patients who needed care.
Solution
We brought the teams together around one shared, data-driven plan for the day. Using integrated patient data, we built a way to see who needed attention most, balanced workloads across the team, and redesigned workflows everyone could follow and commit to.
